A Home Care Nurse provides compassionate and personalized medical care to patients within their homes. As a Home Care Nurse, you will monitor patient health, administer medications, assist with personal care routines, and coordinate with doctors and family members to ensure cohesive patient care. Responsibilities
Provide high-quality, compassionate care to patients in their homes, respecting their individual needs.
Assess patient health status by conducting physical exams and reviewing medical histories.
Develop and implement individualized nursing care plans according to patient needs and physician directives.
Administer medications and intravenous therapies, monitoring patient reactions to treatments.
Perform wound care management, including dressing changes and infection prevention techniques.
Assist patients with daily activities, such as bathing, dressing, and feeding, ensuring their comfort and dignity.
Monitor and record patient health vitals and communicate any significant changes to the healthcare team.
Educate patients and family members on treatment plans, medical procedures, and health maintenance tips.
Coordinate with physicians, therapists, and pharmacists to ensure comprehensive patient care.
Maintain accurate patient records and ensure confidentiality in compliance with medical standards.
Develop a rapport with patients, providing emotional support to enhance the patient experience.
Respond promptly to emergencies, employing critical care skills to stabilize patients.

Requirements
Current state nursing license in good standing without any restrictions.
Minimum of two years of experience in nursing, preferably in home care or a similar setting.
Strong knowledge of medical protocols regarding home care and patient confidentiality regulations.
Exceptional interpersonal and communication skills for interacting effectively with patients and families.
Ability to work independently, managing a flexible schedule with minimal supervision.
Basic computer proficiency for maintaining electronic medical records and communication.
CPR certification and knowledge of first aid protocol for emergency situations.
